Star Trek canon is (probably purposely) vague when it comes to specific sizes and limitations for a good portion of their technology, so we're going to have to make some stuff up based on what we know.

The replicators on board the Enterprise have made instruments in the past, though we're not sure if they can make anything as large as a standard-sized guitar. The only limitations we've found so far seem to be in the object's mass, as the larger the object is the more energy it takes to create it, and the Enterprise only has a finite amount of energy for replicator use before it becomes too "costly" to create an object. The replicators found in everyone's quarters are generally referred to as "small", and typically used to make food or clothing. The Enterprise also has something called a "Replicating Center," which is like a ship's store that can create "large" goods on replicators that look a little more like transporter pads. Knowing all this, we feel fairly confident that a guitar wouldn't be out of the question, so unless canon contradicts us at some future point, here is our official answer:

Yes! The replicators in Ten Forward and each room can only create whole pieces as big as the replicator opening, such as seen here. This is about 3 ft. (or 92 cms) wide x 1 1/2 ft. (46 cms) tall x 1 ft. (30.5 cm) deep. It's probably just a little too small to make a guitar, but it could make something like a ukulele or a mandolin. It can, however, create the components of a guitar that can then be assembled later. The replicators in the Replicating Center, which we're placing on Deck 4 near the ship's stores, can create objects about as large as 2 ft. x 3 ft. x 2 ft., about the size of a large endtable. We're not going to be overly strict about size, so long as it follows the basic guidelines:
- No replicator can create a Starfleet uniform
- No replicator can create any kind of poison
- No replicator can create sophisticated technology (such as an AI, communicator, or small ship), though it can create the basic parts for one


As for the latter part of your question, there are many craftsmen, traders, and businessmen who travel on the Enterprise, so if a character wanted something custom-built they could probably commission it. The ship does have stores where you can acquire many things, and if another player's character has a relevant business aboard, they likely can be reached by an OOC contact post.
